The Chicago Regional Council of Carpenters Board of Trustees cares about you and strives to put programs, benefits and initiatives in place to help you and your family live full, healthy, and happy lives.  The Carpenters Center for Health is state of the art medical facility that provides convenient access to a full range of high quality, healthcare services at no cost to you.  

Who is eligible to use the Carpenters Center for Health?

The Carpenters Center for Health is exclusively for members, and their eligible dependents, ages 2 and up, who are eligible and enrolled in either the Comprehensive Major Medical Coverage or the Comprehensive Medicare Supplement Coverage under the Chicago Regional Council of Carpenters Welfare Fund's Retiree Plan of Benefits.  

What Services are provided at the Health Center?

All of the services listed below are available to you at no cost.  You can utilize the Carpenters Center for Health as your primary healthcare provider or as a supplement to your current medical provider, choosing to take advantage of any or all of the services offered at the Health Center.  You can request that medical documentation of your visit to the Health Center be sent to any of your doctors to ensure coordination of care. 

 Who is on staff at the Health Center?

Dr. Susan Fedinec, D.O., FAAFP, is the Medical Director/Physician.  Dr. Fedinec has over 18 years of experience caring for a diverse population of patients.  She will be joined by a nurse practitioner, registered nurses, a condition management consultant, a health center manager who is also a registered nurse, medical assistants, and other skilled personnel.  The staff at the Carpenters Center for Health will take the time to understand your healthcare needs and provide an integrated and holistic approach to treating not only your medical conditions, but also your overall wellness and disease prevention.

 

Where is the Health Center located and what are the hours of operation?

The Carpenters Center for Health is conveniently located on CRCC’s Lisle Campus at:

         4979 Indiana Avenue, 3rd Floor
         Lisle, IL  60532
 

To meet the needs of your busy schedule, the Health Center is open:

        Monday, Tuesday, Wednesday & Friday:  8:00 a.m. to 5:00 p.m.
        Thursday:  10:00 a.m. to 7:00 p.m.
 

How do I schedule an appointment?

You may call the Health Center during regular business hours at 312-337-4150 or visit www.crcchealthcenter.org to connect to a secure online patient portal to schedule appointments, send secure message to the doctor, view lab results or request prescription refills.  

Will my health information be kept confidential?

The Carpenters Center for Health is operated by Premise Health.  Premise Health is one of the nation’s leading managers of onsite health centers.  They follow stringent clinical guidelines and “best-in-practice” quality controls.  Premise Health will keep your personal health information (PHI) confidential and your data secure.  The Health Center operates in accordance with HIPAA and they work diligently to protect all health records.  They operate through a private network and software systems that can only be accessed by Premise Health employees.  Your PHI cannot be shared without your permission.  You can rest assured that Premise Health will not share any information CRCC staff.
