Retiree Hearing Benefit

For those covered by Groups 50445 and 50446, the Plan will cover a maximum of $1,500 per covered individual for prescribed hearing aid instruments or their repair, once every five (5) consecutive years. Note that coverage is for the device itself. A hearing exam is not covered under the Retiree Plan.

 

Retiree Dental Benefit

Effective April 1, 2016 an insured dental plan through Delta Dental of Illinois is available.

If you do not enroll in the dental plan at the time of retirement, you will be required to wait until next year’s open enrollment period (Generally, open enrollment will be available for April 1st enrollment).
 
If you, your spouse, or your dependent children have dental coverage with another insurance plan you may postpone enrollment in this dental plan until coverage under the other dental plan ends.
 
It is important to realize that, if an individual enrolls in dental coverage, has services, and then cancels coverage before being enrolled in dental coverage for at least one full year, the Trustees prohibit re-enrollment for a period of two years.
 
ID cards:  If you elect to enroll in this dental plan, you will receive a Delta Dental identification card.  When you visit your dentist, be sure to present the card and make sure to let the staff at the dentist’s office know that Delta Dental Plan of Illinois will process your claims. 
 

Finding a network provider:  The Delta Dental PPO & Premier program has the nation’s largest dental network, offering you access to a wide range of participating network dental providers who have agreed to charge lower fees for their services.  Using a Delta Dental network provider will save you money.  Your Plan gives you access to both the PPO and Premier networks.  To find out if your dentist participates in the Delta Dental network, visit Delta Dental of Illinois’ website at www.deltadentalil.com.  To obtain a list of providers in your area, click on “Find a Network Dentist” and select a network. 

Filing a claim for benefits:  All claims for dental services must be filed directly with Delta Dental Plan of Illinois.  Most providers file claims electronically directly to Delta Dental.  Mail paper claims to:  Delta Dental Plan of Illinois, P.O. Box 5402, Lisle, IL 60532.

Retiree Vision Benefit

Effective April 1, 2017 an insured vision plan is available through Delta Dental of Illinois’ DeltaVision® program. The DeltaVision® program uses the EyeMed Insight network of providers.

Finding a network provider:  To locate an in-network provider search the online provider directory at www.deltadentalil.com.  Click on “Provider Search,” and then “Find a Vision Provider.”  You will be redirected to the EyeMed site.  Here, you can key in your zip code and select the INSIGHT network of vision care providers to find a provider near you.
